If you’re in your 40s or 50s and finding that lower back pain is becoming a regular part of your life, you’re not imagining it. This is the age when many people — particularly women — notice that their back feels stiffer, achier, and less forgiving than it once was. The good news is that it doesn’t have to be something you simply put up with.

At The Green Clinic in Edgware, we regularly help patients in this age group understand what’s behind their back and neck pain and, more importantly, what can be done about it.

What Changes in Your 40s and 50s?

Several things happen to the body as we move into midlife, many of which directly impact the lower back. The discs between your vertebrae gradually lose moisture and become less effective as shock absorbers. The joints of the spine can stiffen, and the muscles that support your back may weaken, particularly if you spend long hours sitting or lead a less active lifestyle than you once did.

For women, hormonal changes during perimenopause and menopause play an additional role. Declining oestrogen levels can contribute to a loss of bone density, reduced muscle mass, and increased joint stiffness — all of which place greater strain on the lower back. Research shows that women are significantly more likely to experience lower back pain during the perimenopausal years, often finding that existing niggles become more persistent and new discomfort develops.

Other contributing factors include changes in posture, weight distribution shifting around the midsection, and the cumulative effects of years of repetitive movements or prolonged sitting.

Why Painkillers Aren’t the Answer

It’s tempting to reach for over-the-counter painkillers when your back flares up, and they can help with short-term relief. However, medication only masks the symptoms — it does nothing to address the underlying cause. If your lower back pain keeps returning, there’s usually a reason, and understanding that reason is the first step towards lasting improvement. We see many patients at The Green Clinic who have been managing their pain this way. The difference comes when the root cause is properly identified and treated.

How Physiotherapy Can Help

Physiotherapy is one of the most effective ways to treat and manage lower back pain, particularly when it’s linked to age-related changes. At your first appointment, your physiotherapist will carry out a thorough assessment of your posture, movement, and the specific areas causing pain. From there, a personalised treatment plan is created around your needs.

Your plan may include manual therapy to mobilise stiff joints and release tight muscles in the lower back and hips. These hands-on techniques can provide significant relief, often from the very first session, and help restore range of movement that may have been gradually declining.

Acupuncture is another option we frequently use for lower back pain. Fine needles are placed at targeted points to reduce pain, ease muscular tension, and support the body’s natural healing processes. It works particularly well alongside manual therapy for a more comprehensive approach.

For longer-term management and prevention, Pilates is something we highly recommend. It strengthens the deep core muscles that support your spine, improves flexibility, and corrects postural habits that may be contributing to your pain. Pilates is especially well-suited to this age group because it’s low-impact, can be adapted to any fitness level, and delivers measurable results over time.

Where persistent tightness is a factor, deep tissue massage can also be incorporated to target deep muscular tension, improve circulation, and help the body recover more effectively between sessions.

When Should You Seek Help?

If your lower back pain has been present for more than a few weeks, is getting worse, or is affecting your ability to work, sleep, or enjoy daily activities, it’s time to seek professional advice. The sooner the cause is identified, the quicker we can help you feel better. Many patients see meaningful progress within just a few sessions.
